It’s extremely simple!

Harmon is a telehealth company, which involves the delivery of health care services using electronic communications between a health care provider and a patient who are not in the same physical location. It is therefore essential that we're able to verify your identity and receive comprehensive medical information of your overall health ahead of your initial consultation. This allows us to provide you with our service safely and in accordance with applicable laws and regulations.

Following the booking of your Initial Medical Consultation, please complete the MANDATORY registrations steps below. Please note that without this information, we cannot proceed with your consultation.

  1. Fill in and sign the New Patient Paperwork (Initial Medical Questionnaire and Medical Consent Form). More information here. (https://help.joinharmon.com/en/articles/9503020-where-do-i-find-the-new-patient-paperwork-and-how-do-i-submit-it)

  2. Upload any additional medical document* you might have, such as screening exams’ reports (ex. mammogram, smear test -for female, prostate scan-for male, bone density, ect). *All diagnostic reports/medical letters need to be legible photos or pdfs, containing all the information of the diagnostic centre, date of the exam and patient’s personal details. Any "cut" or "not legible" photo or pdf will be rejected.

This must be completed at least 48 hours ahead of your consultation, any later than this may result in your consultation being cancelled or rescheduled.

Follow the steps below with our mobile phone or PC:

Step 1) Go on the Harmon Website and Log in

Step 2) Log into "My Harmon Health Portal"

Step 3) Click on "Medical Documents"

Step 4) CLICK on "each session" below, to complete them
