How do I submit my ID & Selfie?

Proof of ID

Harmon is a telehealth company, which involves the delivery of health care services using electronic communications between a health care provider and a patient who are not in the same physical location. It is therefore essential that we’re able to verify your identity ahead of your initial consultation. This allows us to provide you with our service safely and in accordance with applicable laws and regulations.

We ask that you submit 2 ID photos ahead of your consultation. Please note that without this information, we cannot proceed with your consultation.

These items must be added to your file at least 48 hours ahead of your consultation, any later than this may result in your consultation being cancelled or rescheduled.

  1. Picture 1 - Photo ID (Passport or driving licence)

    • Please submit a photo of your photo of passport /driving licence. The photo must be clear and readable.

  2. Picture 2 - Selfie with your ID (Passport or driving licence)

    • We ask that you provide a “selfie type” photograph of yourself holding the photo ID close to your face and head so that the wording is clear and readable.

HOW TO UPLOAD YOUR PHOTOS

It’s extremely simple!

STEP 1) Make sure you have both photos taken, ready on your mobile phone or PC

STEP 2) Log into your “My Harmon Health Portal” and CLICK on “Medical Documents”

STEP 3) Once you are in the “Onboarding Documents’ Page”, CLICK on “ID CHECK”

STEP 4) Once you are in the “ID check (Photo & Selfie)’s Page”, CLICK on “Drop your ID here or Browse” and on “Drop your Selfie Photo here or Browse” to upload the two photos
