New Patient Paperwork

Harmon is a telehealth company, which involves the delivery of health care services using electronic communications between a health care provider and a patient who are not in the same physical location. It is therefore essential that we receive comprehensive medical information of your overall health ahead of your initial consultation. This allows us to provide you with our service safely and in accordance with applicable laws and regulations.

We ask that you fill in and submit our New Patient Paperwork. Please note that without this information, we cannot proceed with your consultation. You will be able to discuss any question you might have on the Medical Consent Form during your initial consultation with the Doctor.

The New Patient Paperwork must be completed at least 48 hours ahead of your consultation, any later than this may result in your consultation being cancelled or rescheduled.

WHERE TO FIND AND HOW TO SUBMIT THE NEW PATIENT PAPERWOK

It’s extremely simple!

STEP 1) Log into your “My Harmon Health Portal” and CLICK on “Medical Documents”

STEP 2) Once you are in the “Onboarding Documents’ Page”, CLICK on “New Patient Paperwork”

STEP 3) Fill in the Harmon Initial Medical Questionnaire

STEP 4) Read the Medical Consent Form, signed and submit the form
